ITIN Document Checklist

For new ITIN applications and renewals. Bring originals for every person applying. We examine them in person, make certified copies for the IRS and hand your originals back before you leave.

Passport = stand-alone document. A valid, unexpired passport proves identity and foreign status by itself. Our CAA examines it, makes a certified copy and sends the copy to the IRS with your Form W-7 and the federal tax return you provide. Your passport stays with you.

1. Every person applying

  • Valid, unexpired passport
  • No U.S. visa in the passport? Your I-94 arrival record too (print it free at i94.cbp.dhs.gov)
  • No passport? At least two current documents, one with a photo and one showing foreign status (for example, a national ID card and a civil birth certificate)
  • The federal tax return the ITIN is for, or ask us to prepare it (save $50)
  • Your U.S. mailing address
  • Each person applying comes to the appointment in person

2. Spouse

  • Spouse’s valid passport, or two current documents if no passport
  • Both spouses attend in person
  • Your joint tax return, if you file together

3. Dependents (children or relatives you claim)

  • Passport
  • Civil birth certificate
  • If the passport has no U.S. entry date, U.S. residency records:
Under 6: U.S. medical or daycare records, or a lease, utility or bank statement showing the child’s name and U.S. address
Ages 6 to 17: U.S. school records (report card or transcript), or a lease, utility or bank statement
18 and older: U.S. school records, lease, utility or bank statement
  • School and medical records must be originals or certified copies from the school or doctor’s office. The IRS receives the record itself and returns it by mail.
  • Children who live in Mexico or Canada do not need U.S. residency records

4. Students and exchange visitors (F, J, M visa)

  • Passport with your valid U.S. visa
  • Form I-20 or DS-2019
  • Letter from your school’s international office (DSO) confirming enrollment
  • Social Security denial letter, if you received one
  • Form 1042-S or scholarship letter, if applying under an exception

5. Renewals only

  • Your old ITIN letter from the IRS
  • Any IRS notice about your ITIN (for example, CP48)
  • Proof of a name change, if your name is different now (marriage certificate or court order)

6. Helpful to bring

  • W-2s, 1099s and other income forms for the tax return
  • Last year’s tax return
  • Any letter you received from the IRS
