Birth certificate amendment,
prepared and filed right the first time

We prepare your California birth or marriage record correction, notarize the sworn statement in our office, and mail a complete packet to Vital Records.

What does an amendment fix?

A California birth certificate amendment (or marriage record amendment) corrects an error on the official record, such as a misspelled name, a wrong date, or a parent’s details. Requests go by mail to CDPH Vital Records, which reports 9 to 11 weeks for complete requests.

Why it can’t wait

One wrong letter on a birth certificate
can stall a passport or REAL ID

Agencies match your documents letter for letter. A misspelled name or wrong date of birth can put a new passport application, REAL ID, Social Security, or immigration filing on hold until the record is fixed.

The hospital can’t change it, and the county Clerk-Recorder doesn’t amend vital records. Every correction goes to the state by mail, and an incomplete request is where people lose months: CDPH reports it takes about 12 to 14 weeks just to send a letter asking for missing documents.

A complete birth certificate amendment packet moves on the first review. That is what we prepare.

Common questions

Birth and marriage certificate amendment
questions, answered

How do I amend a birth certificate in California?
Most corrections use the Application to Amend a Birth Record (VS 24B) with a notarized Sworn Statement (VS 20) and supporting documents, mailed to CDPH Vital Records. Updates after a court-ordered name change use form VS 23. We prepare the packet, notarize your statement, and mail it for you.
How long does a California birth certificate amendment take?
CDPH reports an average of 9 to 11 weeks once it receives a complete request. Incomplete requests take about 12 to 14 weeks just to receive a letter asking for missing documents, which is why we review every packet before it is mailed.
How much does it cost to amend a birth certificate in California?
The state fee is typically $26 to $36 and includes a certified copy of the amended record. Our preparation service is a flat $200, so most clients pay $226 to $236 in total.
Can I correct a mistake on my California marriage certificate?
Yes. Errors on a public marriage record are corrected with the Application to Amend a Marriage Record (VS 24C), signed by two people who know the facts. Changing the date or place of marriage requires the officiant as one signer, and confidential marriage records are amended through the county clerk where the license was issued.
Does the sworn statement for an amendment need to be notarized?
Yes. CDPH requires a notarized Sworn Statement with most birth amendment requests. Our in-house notary handles it at your appointment, so you don’t need a separate stop.
Can Santa Clara County amend my birth certificate at the county office?
No. The Santa Clara County Clerk-Recorder does not correct or amend birth, death, or public marriage records. Those requests go to the California Department of Public Health by mail, and we prepare and send them for you.
Will I get a corrected copy after the amendment is approved?
Yes. The amendment becomes part of your official record, and the state fee includes one certified copy of the amended record. You can then use it for a passport, REAL ID, or an immigration filing.
Can you help if I live outside Santa Clara County?
Right now this service is for Santa Clara County residents only, because your sworn statement is notarized in person at our San Jose office.
